You are hereby notifed that an action has been commenced against you in the Circuit Court of Jackson County, State of Missouri, at Kansas City, the object and general nature of which is: PETITION FOR ADOPTION, NAME CHANGE, AND TERMINATION OF PARENTAL RIGHTS OF MINOR CHILD.
You are further notified, pursuant to Section 211.462(2) R.S.Mo of your right to have counsel, and if you request cousel and are financially unable to emply counsel, counsel shall be appointed by the Court.
The names of all parites to said action are stated above in the caption hereof and the name and address of attorney for Petitioner is: KELLY KEEFE, 3500 NE RALPH POWELL ROAD, SUITE A., LEE'S SUMMIT, MO 64064.
You are further notifed that, unless you file an answer or other pleasings or shall otherwise appear and defend against the aforesaid petition within 45 days after the first date of publication, a judgment by default will be rendered against you.
